dc.description.abstract | La stratification pronostique des leucémies aigues lymphoblastiques B repose largement sur l’identification d’anomalies génétiques récurrentes. L’objectif principal de notre travail était d’évaluer les performances d’une technique de Reverse Transcription Multiplex Ligation Probe Assay (RT-MLPA) dans l’identification en routine des 4 principales anomalies génétiques récurrentes au diagnostic des LAL B (BCR::ABL1, ETV6::RUNX1, TCF3:BX1, KMT2A:AF4). Ces anomalies ont pu être mises en évidence avec une bonne sensibilité (95.6%) et une bonne spécificité (100%). D’autre part dans les LAL B, il existe une entité récemment mise en évidence: les « LAL B Phi-like ». Elles rassemblent plus de 60 anomalies génétiques ayant en commun une hyperactivation des voies de signalisations intracellulaires associées à des tyrosines kinases et sont associées à un mauvais pronostic. Afin d’évaluer l’apport de la RT-MLPA dans le diagnostic de ces LAL Phi-like, nous avons étudié de manière rétrospective une cohorte de 80 patients de plus de 25 ans, diagnostiqués au CHU de Bordeaux entre janvier 2008 à janvier 2018. Dans cette cohorte l’association de la RT-MLPA et d’une FISH ciblée sur IGH, EPOR et CRLF2 a permis d’identifier 8 patients (10% de la cohorte, et 22% des patients sans anomalie cytogénétique récurrente) avec une anomalie associée aux LAL B Phi-like, en majorité des réarrangements de CRLF2. Cette étude a permis d’adopter la RT-MLPA comme technique de routine au laboratoire d’hématologie du CHU de Bordeaux, pour tout diagnostic de LAL B, de l’enfant comme de l’adulte, au diagnostic et à la rechute, en association à la cytogénétique (caryotype, FISH KMT2A) et a permis de remplacer les 4 RT-PCR qualitatives. | |
dc.description.abstractEn | The prognosis stratification of B-cell acute lymphoblastic leukemia is largely based on the identification of recurrent cytogenetic abnormalities. The main objective of our work was to evaluate the performance of a Reverse Transcription Multiplex Ligation Probe Assay (RT-MLPA) for the routine identification of 4 main recurrent cytogenetic abnormalities in the diagnosis of B-cell ALL: BCR::ABL1, ETV6::RUNX1, TCF3:BX1, KMT2A::AF4. These abnormalities could be highlighted with good sensitivity (95.6%) and good specificity (100%) Moreover in these B-cell ALL, there is an recently highlighted entity: Phi-like (or BCR::ABL1-like) B-cell ALL. More than 60 genetic abnormalities have been described, leading to the hyperactivation of intracellular signaling pathways associated with tyrosine kinases, and are associated with poor prognosis. In order to evaluate the contribution of RT-MLPA in the diagnosis of these Phi-like ALL, we retrospectively studied a cohort of 80 adults over the age of 25 diagnosed from January 2008 to January 2018 at the Bordeaux University Hospital. The combination of RT-MLPA and FISH targeted on IGH, EPOR and CRLF2 identified 8 patients (10% of the cohort, and 22% of patients previously classified without recurrent cytogenetic abnormality) with a rearrangement associated with Phi-like B-ALL, mostly CRLF2 rearrangements. These study made it possible to adopt RT-MLPA as a routine technique for diagnosis and relapse of B-cell ALL, and to replace the 4 qualitative RT-PCR. | |
