En | Daily practice should be based as much as possible on well-designed clinical research. Clinical trials are the main source of data but should not be the only one. Data from cohorts are also essential, particularly for populations such as older patients who are too often excluded from trials. Indeed, a major difficulty is heterogeneity of studied populations, not only at the geriatric level but also at the cancer level. Results may highly vary if they are drawn from a first-line treatment population or from relapsing/progressive patients, from referral hospitals or from the community setting. Different populations may lead to different trial results, which is why evidenced-based medicine pleads not to rely on single trial results. Therefore, to draw appropriate conclusions and ensure transfer to routine practice, this difficulty must be mastered. | |
