La réhabilitation implantaire de la mandibule postérieure partiellement édentée est un problème clinique courant. Pour pallier à une atrophie mandibulaire postérieure, l'approche idéale consisterait à augmenter verticalement l'os de manière prévisible et réussie, avec des résultats stables dans le temps. L'ostéotomie segmentaire, également connue sous le nom de "technique sandwich » a été décrite pour la première fois par Schettler en 1976 et consiste à soulever un segment ostéotomisé coronal de la mandibule qui est encore attachée au périoste lingual et l'interposition d'une greffe de bloc osseux. Cette technique garantit un apport vasculaire plus important, provenant du périoste lingual et de l'os résiduel à la greffe d'incrustation. Nous traitons ici le cas de deux patients dont la mandibule postérieure est sévèrement atrophiée (Cawood 5) rendant impossible la pose implantaire. Nous utilisons comme thérapeutique la technique d’ostéotomie segmentaire afin d’obtenir un gain de hauteur osseuse vertical suffisant à la pose ultérieure d’implants.Read less <

The implant rehabilitation of the partially edentulous posterior mandible is a common clinical problem. To overcome posterior mandibular atrophy, the ideal approach would be to vertically augment bone in a predictable and successful way with stable results over time. Horizontal osteotomy with the interposition of bone in the form of a sandwich, also known as the “inlay technique,” was first described by Schettler in 197621 and involves raising a coronal osteotomized segment of the mandible that is still attached to the lingual periosteum and interpositioning a bone block graft. This technique guarantees a greater vascular &supply, coming from the lingual periosteum and the residual bone to the inlay graft. In this case, we are treating two patients whose posterior mandible is severely atrophied (Cawood 5), making implant placement impossible. We use the segmental osteotomy technique to obtain sufficient vertical bone height for subsequent implant placement.Read less <
